core 100 mg problems

I have a issue's with the with the spool developing play and coming loose, it's happened 4 times now, I have to open up the left side plate and re-tighten the red brake case, any idea's why this keeps coming loose on it's own?
I have two other 100 and haven't had this happen to those, Yet!

Use a pair of needle nose pliers and insert the tips into the holes drilled in the brake case. Back the cast control cap off to make sure the spool is not contacting it prior to tightening the case. I make sure it is hand tight then snug it a little with the needle nose. I have never had one come loose.
