Engine Maintenance Log Form/Book

Has anyone ever designed a maintenance log for their motors? I'm wanting to either purchase or build my own. I've shopped around and found nothing. I want a document/log book of my engines year built, make, model, serial number, maintenance completed, maintenance next due, types of oils, filter part numbers, gas type/additives, etc.

Not a bad idea just wondering how many anglers actually still work on their own motors today with how complex outboards have become ?
