Do I gain anything going full synthetic over just using the semi- synthetic 2 stroke oil ?
I'm using it an older 2009 Mercury Opti 90HP

Both are great choices obviously the semi-synthetic is a little easier on the wallet.
To directly answer your question the full synthetic has a low ash additive to reduce carbon and emissions. The full synthetic has lasting lubricty additives to help protect even when the engine isn't running.
I'm not a scientist. I just know there is almost no carbon build up inside of my prop.
It's really up to you if that's worth the extra few bucks.

At my stage in life, the money is not that important. But having my motor last with the least chance of trouble is.
